What is Male Infertility?

Male infertility means a man’s body struggles to make healthy sperm, making it hard to get a woman pregnant. Imagine sperm as tiny swimmers racing to reach an egg—if there are too few, they’re slow, or shaped oddly, pregnancy becomes difficult. Surprisingly, 40% of couples struggling to have a baby face male infertility issue. Worldwide, over 60 million couples deal with infertility, making it a common but often misunderstood problem.

How Common is Male Infertility?

About 1 in 10 men experience infertility. Causes range from hormone problems and genetic issues to blockages in sperm pathways. Sometimes, doctors can’t find a reason (called “unknown causes”). Unhealthy habits like smoking, poor diet, or untreated diabetes can also lower sperm quality.

Causes of Male Infertility

  1. Hormonal Imbalances: Low testosterone or other hormones.
  2. Genetic Issues: Inherited conditions like cystic fibrosis.
  3. Blockages: Sperm can’t exit the body due to scar tissue or missing tubes.
  4. Lifestyle Choices: Smoking, alcohol, obesity, or stress.
  5. Health Problems: Diabetes, infections (like mumps), or cancer treatments.

Symptoms of Male Infertility

Most men don’t feel sick, but signs include:

  • Swelling or pain in the testicles.
  • Low semen (fluid) during ejaculation.
  • Trouble maintaining erections or ejaculating.
  • Small, firm testicles.

Note: Many men only discover infertility after trying to conceive.

Tests for Male Infertility

Doctors use these tests:

  1. Semen Analysis: Checks sperm count, movement (motility), and shape (morphology).
  2. Hormone Tests: Measures testosterone, FSH, and LH levels.
  3. DNA Fragmentation Index (DFI): Tests sperm DNA health.
  4. Ultrasound: Looks for blockages.

Healthy Sperm Numbers (WHO Standards 2025):

  1. Volume: At least 1.4 ml (½ teaspoon) of semen.
  2. Concentration: 16 million sperm per ml.
  3. Total Count: 39 million per ejaculation.
  4. Movement: 42% moving; 30% swimming straight.
  5. Shape: 4% normally shaped.

Azoospermia: No Sperm in Semen

Types:

  1. Obstructive Azoospermia (OA): Sperm is made but blocked from exiting.
    • Causes: Injury, surgery, or birth defects.
    • Treatment: Surgery to unblock tubes.
  2. Non-Obstructive Azoospermia (NOA): Body doesn’t make enough sperm.
    • Causes: Genetic issues, hormone problems, or undescended testicles.
    • Treatment: Hormone therapy or sperm extraction.

Treatments for Male Infertility

  1. Surgery: Fixes blockages or retrieves sperm directly from testicles (TESE, PESA).
  2. Medications: Balances hormones or treats infections.
  3. Assisted Reproductive Technology (ART):
    • IUI: Washed sperm is placed directly into the uterus.
    • IVF: Eggs and sperm are combined in a lab; embryos are implanted.
    • ICSI: A single sperm is injected into an egg (for severe cases).
  4. Lifestyle Changes: Quit smoking, eat nutritious foods, exercise, and reduce stress.

Breaking the Stigma Around Infertility

In many cultures, infertility is blamed on women, but men are equally likely to have issues. We need to:

  • Talk Openly: Share stories to reduce shame.
  • Educate: Teach that infertility is a medical problem, not a personal failure.
  • Support Research: More funding can improve treatments.
